Privacy and Security of Personal Health Information

This policy is to provide information to you our client, on how your personal information is collected and used within our practice and the circumstances in which we may share it with third parties.

Armajun AHS complies with Federal and State privacy regulations including the Privacy Act 1988, the Privacy Amendment (Private Sector) Act 2000, the Privacy Amendment (Enhancing Privacy Protection) Act 2012 and the Privacy Regulations 2013.

When you register as a client of our practice, you provide consent for our staff to access and use your personal information so they can provide you with the best possible healthcare. Only staff who need to see your personal information will have access to it. If we need to use your information for anything else, we will seek additional consent from you.

We collect, use, hold and sometimes share your personal information so we can provide and manage your healthcare needs. We also use this information for business related activities, such as financial claims and payments, practice audits and accreditation and business processes (eg staff training).

The Personal information we collect includes:

  • Name, date of birth, address and contact details;
  • Medical information - medical, social and family history, allergies, medications, immunisations, risk factors etc;
  • Medicare number, healthcare identifiers and health care fund details.
